AMBULATORY BLOOD PRESSURE MONITOR

 Borderline high blood pressure
 If patient and doctor can’t keep blood pressure under control
 If patients have blood pressure problems caused by other medicines
 If patients is pregnant and have high blood pressure
 If patients have fainting spells
 If patients have “white-coat hypertension”

IMPORTANT NOTES:
>> Bring you update health card, current medications and this referral form.
>> Wear comfortable clothing and running shoes for STRESS ECHO and STRESS TEST appointment time
>>No caffeine products should be taken ONE hour prior to STRESS ECHO and STRESS TEST appointment time
>>Hold Beta blocker and Calcium Channel blocker for 48 hrs based on physician’s advice (if clinically feasible)
>>Do not put oil, lotion on the chest area
>>Please arrive 10 minutes early prior to you appointment time
